Services

 

The services offered by Wilkens Rehabilitation and Case Management Services can include; and are not limited to:

 

  • Catastrophic and Non-Catastrophic Injury Case Management of pediatric, adult and senior clients who have sustained traumatic brain and spinal cord injuries, multiple fractures, and issues relating to complex care.
  • Facilitating transition to community from hospital setting; recognized for timely response and professionalism. Adept at consulting with hospital staff, physicians and specialists in order that all documentation including medical reports and clinical notes are reviewed and documented in detail therefore supporting ongoing treatment planning.  Medical recommendations and physician orders are also reviewed prior to the client’s discharge from hospital in order that there is a seamless transition medically from the hospital setting to the client’s home or long-term care setting.
  • Liaising with and providing timely thorough reports to legal councel, insurers and rehabilitation team members ensuring a cohesive approach to care
  • Assessing and evaluating service needs and priorities for current support requirements
  • Developing, implementing and coordinating a cohesive community rehabilitation team
  • Integrating short and long term goals and needs
  • Measuring, evaluating, and monitoring progress to achieve specified outcomes
  • Working to ensure timely and reasonable access to goods and services
  • Direct support in the areas of education and the nature of acquired brain injury as well as support and education to caregivers and/or the client’s family and support network as appropriate
  • Encouragement, emotional support and motivation
  • Referral to and accessing other desired/required community and primary health care needs
  • Conducting file reviews of previous interventions to provide recommendations for further care
